Person-Centered Planning (PCP)

The Plan Becomes Your Map to Pursuing Meaningful Goals

At Wings of Wisdom, we believe every person deserves a plan that’s built around who they are and what matters most to them. That’s why we use something called person-centered planning. It is a supportive, team-based process that helps individuals with intellectual and developmental disabilities (IDD) and their families make real, meaningful progress toward a good life.

Instead of using a one-size-fits-all approach, we focus on each person’s hopes, strengths, and what they want for their future. That could mean finding a job, building independence, improving health and medical support, making friends, or being more involved in the community; whatever matters most to the person.

Person-Centered Planning (PCP) Overview

  • PCP is for all ages.
  • At a Person-Centered Planning gathering, people who love and care about the person work together to design a vision for the individual’s life.
  • PCP is a way for you to say what is important to you and what is important for you.
  • A person-centered plan helps you define how to make the things you want in your life happen.

We are here to help you plan for your best future!
